What Is an Electronic Signature?
There are several definitions to clarify.
  • A digitized signature is an image of a pen-to-paper signature.
  • An electronic signature can be an electronic sound, symbol, or process associated with a record. It is a mark added to a document to indicate intent to sign.
  • A digital signature is a form of encryption that tightly binds documents to a unique number or electronic "fingerprint.” Private companies called certification authorities provide private/public software key combinations used to encrypt and decrypt documents, and they issue "certificates" to bind the public keys to the key holders. However, certification business practice lacks national governance guidelines.

Electronic signatures are legally valid in all 50 states, and the security standards of electronic signatures are addressed in the security and privacy provision of the Health Insurance Portability and Accountability Act (HIPAA).

HIPAA transactions do not require a digital signature at this time. However, HIPAA-covered entities that use electronic signatures for transactions must adhere to U.S. Department of Health and Human Services electronic signature standards.

The security of Electronic Medical Records can be enhanced by the addition of digital signatures that guarantee:

 
  • Non-repudiation assurance- that the signer cannot deny signing the document in the future
  • User authentication - verification of the signer's identity at the time the signature IS generated, and
  • Message Integrity - certainty that the document has not been altered since it was signed.
Most current EMR applications use some form of electronic signature to append the provider's name, marking the encounter as closed.

The costs of adding digital signature to EMR mandates serious examination of the business case for digital signature within an EMR as compared with transactions such as electronic prescriptions. At present, there is no regulatory requirement for digital signature of an EMR.
Hand Writing Recognition and EMR

Using Hand Writing (HW) Recognition with EMR, on a TabletPC
Tablet PCs (TPCs) are great for EMRs. Tabletpcs are mobile computers that can run any EMR that a desktop PC can run. At a very basic level the difference is a stylus replaces the mouse. The stylus can be much more friendly and intuitive with an EMR. Additionally, new TPCs have built in voice and handwriting recognition as well as inking that recognizes whatever you write!
A Tablet PC facilitates stylus input, with a simple point and click method.

The best way to store data and introduce Hand Writing (HW) is within a "Dialog Box". Dialog boxes are boxes that pop up for user input when a user clicks on hyperlinked text. When the dialog box opens up, you simply click on your desired choice or start writing in the box and your handwriting will be converted to text. You don't have to use the Tablet Input Panel (TIP). You can write directly into the dialog box and your writing will be converted to text. Accuracy is excellent (98%) as you are seldom writing more than a few words or a sentence.

Additionally, TPC users can use the Tablet Input Panel (TIP) that comes with the TPC or use third party handwriting recognition, such as Pen Office
